DAP+ Telehealth model is an additional option for patients, GPs, and GP practices. Patients with diabetes can be seen in a video consultation at their GP practice with their GP and Diabetes specialists.

DAP+ telehealth is a culturally safe alternative that enables support from families or carers and eliminates access and transport difficulties for patients.  
Like DAP+ Case Conferences, DAP+ telehealth involves the endocrinologist, diabetes educator, GP and practice nurse involved in the video consultation at one time and with this DAP+ telehealth appointments are made at the GP practice and DAP+ team.
