How to get involved in DAP++

If you are a patient living with diabetes and would like to participate in DAP+ Case Conferencing Clinics, ask your GP if they are registered with DAP+.

What if your GP isn’t registered? Let them or the general practice know to express their interest using the below form.

Personalise your path to better diabetes care with DAP+, a specially designed diabetes care model just for you. 

Delivered as a partnership between the Hunter New England and Central Coast Primary Health Network, Hunter New England Local Health District, Hunter Medical Research Institute and The University of Newcastle, DAP+ is available via local general practitioners supported by a team of diabetes experts including endocrinologists, dietitians, and diabetes educators.  Since 2015 the program has been adopted by more than 560 GPs and 250 practice nurses in 160 general practices, supporting 4,000 patients.

That's why DAP+ offers one-on-one consultations with both your primary care team and a specialist team. Together, they will develop an individualised care plan that considers your specific goals and challenges.
