DAP+ is currently available to GP practices and Aboriginal medical services in the Hunter New England region of NSW.

Register your interest to participate in DAP+ at your practice and a team member will be in touch to visit you and: 

-Discuss the program workflow of case conferencing integrating care with your team. 

-Book dates that suit your practice and clinicians. 

-Assist in the selection of patients. 

-Provide pre-clinic education for nurses.
