DAP+ celebrates a decade of transforming diabetes care across the Hunter and New England region.  From humble beginnings, DAP+ has grown into a gold-standard model of integrated, community-based care - empowering over 700 GPs, 200 nurses, and thousands of patients through innovative case conferencing, education, and support.

DAP+ has launched the Medibus - a mobile health initiative designed to bring diabetes education, screening, and support directly to communities.  DAP+ needs your help to equip the Medibus with essential tools, like cooking gear for nutrition education, portable AV equipment for outdoor learning, podiatry tools, yarning mats, and more. 

Every item purchased will proudly display your organisation’s name as a valued supporter of better health outcomes for adults living with diabetes.
