DAP+ Wins Top Award for Primary Care Innovation

The Diabetes Alliance Program Plus (DAP+) has been recognised as a leader in improving community health, taking out the Primary Care Innovation Award at the 5th annual 2025 Hunter New England and Central Coast Primary Health Network (HNECC PHN) Primary Care Quality and Innovation Awards. 

DAP+ was selected as a finalist in three award categories, including First Nations Health, Technology in Health, and Primary Care Innovation. The team proudly claimed the win in the Primary Care Innovation category.

The Primary Care Innovation Award recognises organisations, clinics, teams or individuals that demonstrate outstanding innovation in primary care. DAP+ was commended for its evidence-based model of care, which strengthens general practice capacity and improves access to specialist diabetes care across the region.

Through face-to-face and virtual multidisciplinary case conferencing, an urgent diabetes and endocrine advice line, lifestyle modules, practice performance appraisals, quality improvement initiatives, and RACGP-credentialed diabetes masterclasses, DAP+ is enhancing both health outcomes and equity of access for people living with diabetes.
